Be sure that the shipping address noted is listed correctly - this is where the needed publications and information packet will be sent.

Once you have submitted the registration form and your payment has been processed, official enrollment in the program will be complete. Copies of the needed publications and the program information packet will be sent to the shipping address. Please allow 10 to 14 days for materials to arrive from your enrollment date.
